Providing professional skilled nursing care to patients in their own homes. Our rns provide medical services to our patients with courtesy, respect, and warmth. We offer nursing services on both short-term (during a recovery from an illness, accident, or surgery, for example) and long-term (for patients with chronic conditions) bases.

supplies, including but not limited to:

Transitional Care Management

We use disease management interventions targeted towards
patients transitioning from one care setting to another.
• Home visits by a Nurse Practitioner, who delivers
education, reinforces self-care instructions, reconciles
medications, and performs physical examination
• Implement evidence-based treatment guidelines and lead
home health multidisciplinary team
• Aid in reducing hospital readmissions and other avoidable
medical expenses

LSVT: Therapy for Parkinson's Disease

We offer a new approach to the treatment of Parkinson’s
disease involving the concept of neuroplasticity.

  • LSVT certified practitioners
  • Four LSVT-LOUD and/or LSVT-BIG sessions a week, for four consecutive weeks
  • Sessions are individual one-hour treatment sessions
  • Daily homework practice (all 30 days of the month)
  • Daily carryover assignments (all 30 days of the month)

Lymphedema Therapy

Our lymphedema treatments focus on reducing swelling,
controlling pain, and self-management.

  • Lymphedema certified practitioner
  • Education on the condition
  • Compression bandaging/garments
  • Manual lymphatic drainage therapy
  • Development of at-home self-management program

Telemedicine & MedMinder

We use telecommunication and information technology to
provide clinical healthcare from a distance.

  • Telehealth – high-risk patients receive daily phone calls
    from a registered nurse
  • Telemonitoring – remote vital sign monitoring 24/7
    (BP, weight, BG, oxygen saturation, & HR)
  • MedMinder – medication dispenser that locks and
    notifies patient when it’s the correct dosage time
