LPN for Private Duty –

Intermittant or PRN Staffing

Job Description:

You will administer skilled nursing care to private duty patients on an intermittent or PRN basis in the home, as outlined in each patient’s care plan established by the physician, supervising nurse, or case manager. Your specific duties will include:

• Assessing the patient's condition and nursing needs
• Setting goals
• Evaluating the patient's symptoms
• Reporting reactions to treatments of drugs and changes in physical and emotional condition to the case manager
• Instructing and counseling patients and their family members regarding the nursing care
• Providing continuity of care by working cooperatively with nursing and other team personnel to maintain the standards of professional nursing in the home setting.

You will be responsible to the Supervisor of Family Care at Home, our private duty division.

Education:
You must have a current valid LPN license and a valid driver's license. Home health experience is recommended but not required.

Required Knowledge:
You should have excellent nursing skills, a high level of initiative, and good communication skills. You should also be able to work well with others and enjoy working with elderly and ill people.
