LPN (Licensed Practical Nurse) Hospice Shifts-Mobile & Baldwin Counties

Home hospice care is a holistic mode of care and is well suited for people looking for purpose in what they do. You will be responsible for managing the care of patients and working with their families, while coordinating with a multidisciplinary care team and the attending physician. You will administer skilled nursing care to patients on an intermittent or PRN basis in the home, as outlined in each patient’s care plan established by the physician, supervising nurse, or case manager. Your specific duties will include:

• Assessing the patient's condition and nursing needs
• Setting goals
• Managing symptoms and medications
• Coordinating with the multidisciplinary care team
• Reporting reactions to treatments of drugs and changes in physical and emotional condition to the case manager
• Instructing and counseling patients and their family members regarding the nursing care

You will be responsible to the Director of Nursing.

You must have a current valid LPN license and a valid driver's license. At least one year's nursing experience in required; home health experience is recommended but not required.

Required Knowledge:
You should have excellent nursing skills, a high level of initiative, and good communication skills. You should also be able to work well with others, have a high level of initiative, and enjoy working with elderly and ill people.
